All patients are eligible to register for on-line access.
Every applicant will need to complete one of our on-line access forms which are available from reception or from the page on our website explaining about detailed coded on-line access and access to test results. Once completed, please bring this to reception with two forms of identification. One should be ID with a photograph and one should have your current address. For children, we are happy for parents to vouch for their identity.
Once handed in to reception, we will process your application. This normally takes a week. It is your responsibility to come in to reception to pick up your unique patient number and the log in details.
If a child has been registered for on-line access by their parents, once they reach 16 years, our Practice Policy from 1st April 2016, is that they will have to register again in their own right. We will write to them to inform them of this before they reach 16years old.
