All patients are eligible to register for on-line access.
Every applicant will need to complete one of our on-line access forms which are available from reception or from the page on our website explaining about detailed coded on-line access and access to test results. Once completed, please bring this to reception with two forms of identification. One should be ID with a photograph and one should have your current address. For children, we are happy for parents to vouch for their identity.
Once handed in to reception, we will process your application. This normally takes a week. It is your responsibility to come in to reception to pick up your unique patient number and the log in details.
Children under 16 years are not able to register for patient access, but our Practice Policy is that they will be able to register in their own right after that age. They will need to present their own contact details and forms of identity to be able to register after the age of 16.
