We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by completing a form available from reception. When you register, you will be asked to provide 2 forms of identification. One of these should have your address in our area on it, the other should be a passport, driving licence or birth certificate. Once you have handed in the paperwork to register with the Practice, there will be a 48 hour turnaround before you will be able to make an appointment with a doctor.
PLEASE MAKE SURE YOU REGISTER WITH A DOCTOR AS SOON AS YOU MOVE INTO THE AREA. DO NOT WAIT UNTIL YOU NEED TO SEE A DOCTOR!
From 2nd October, the new registration form will include supplementary questions for overseas visitor patients to declare when they have a non-UK European Health Insurance Card (EHIC), Provisional Replacement Certificate (PRC) or S1 form. These questions will be used to determine an overseas patient’s eligibility to free NHS Secondary healthcare. All new patients will be given a leaflet to explain this process.
